Getting started with Home Care
1. Complimentary Consultation
When you contact Abode Independence, a discussion will take place to confirm a few details about you and set up a time to complete an in-home interview and assessment.


2. Interview and Assessment
The goal of the in-home interview and assessment is to better understand you and your needs. To ensure we provide you with successful support, we must understand you as a person: the ins and outs of your personality, what activities you require support with, your health conditions and their trajectory, how you conduct the business of your home, and your daily-weekly-monthly routines. Once this information is gathered, a custom Service Plan is created for you.
3. Service Plan
The Service Plan is a customized written strategy created in collaboration with you and our health care staff. When appropriate, your personal physician may provide input on the Service Plan as well. The Service Plan provides personal details about you, the list of services to be provided to you (includes companion and personal care), and descriptions of how service tasks will be performed.


4. Service Agreement
The Service Agreement is your contractual agreement to accept Abode Independence as your home care provider. Included is the consent to receive services, payment obligation, policies and procedures for the services being provided, and is signed by you and one of the owners of Abode Independence Home Care. Once the Service Agreement is accepted, we then secure your service deposit.
5. Care Partnership
At Abode Independence, care is provided by our trained and compassionate Care Partners. After being chosen through a rigorous recruiting process, our Care Partners are given extensive training on how to be an extension of you in support of your graceful aging in the comfort of your own home. They partner with you to implement the details of your customized Service Plan.
